If you’ve upgraded to the latest Wordpress release 2.1, you should have noticed that several contact form plugins stopped working properly. The contact form was not displayed. I would therefore recommend you the Secure and Accessible PHP Contact Form that works smoothly with the 2.1 version. See a working version at my contact page. If you would like something easier,copy here and paste there, my how-to contact form for blogger is a must(Thanks to Kyu for reminding me). Follow the instructions that will guide you in putting the appropriate code and you should be up with a great contact form in seconds.

Ah I didn’t know there were problems! Although to be fair I have only just installed a contact form onto my contact page. It doesn’t surprise me that there have been problems as I have noticed there have been quite a few plugin compatibility problems with WP2.1.
There are no problems with the excellent Dagon Design Form Mailer plugin. I had to swap to it when another plugin stopped working on my theme on Wordpress 2.0.4 and it’s worked perfectly since.

yeah I think you should also check with your theme to make sure whether everything works fine. I think there have been issues with categories and such stuffs….
but probably famous themes like k2 got them fixed in their latest release…

I tried that one with WP 2.1 but still it wasn’t working..The only one working with my wordpress installation is the above plugins…I tried checking the WP-contact form site but it seems to be down since several days.
